General Ed Teacher Mentor Evaluations

This Google form can be used to gain insight from the general education teachers into how the mentors are doing in the general education classes where they are supporting their mentee. Fully editable to fit the needs of your school.

Director of Special Ed Mentor Evaluations

This Google form is designed to help give the Director of Special Education information about how the peer mentor is providing support to their mentee during class during a pop-in evaluation. This form is intended to be used by the Director of Special Education to get a "snapshot" of how the mentor is providing support to his or her mentee in the class. This is a confidential form that will only be used and seen by the Director of Special Education. Information provided in this form may inform conversations that may need to be had with the mentor regarding engagement, behavior or responsibilities.

Mentee Feedback

This Google form is designed to help give the Director of Special Education information about how the peer mentor is providing support to their mentee during class from the perspective of the mentor's mentee. This is a confidential form that will only be seen by the Director of Special Education. Information provided in this form may inform conversations that may need to be had with the mentor regarding engagement, behavior or responsibilities. Depending on the students support needs they may need help navigating and completing this form with the help of a teacher.

ABCs Behavior Chart

This Google doc, which is fully editable to fit your school's needs, has drop downs for setting, antecedent ,  behavior and consequence as well as the function of the behavior. This can be used to collect data about student behavior and help teachers and school staff identify behavior patterns.
